Usually you do not send the claim to CareFirst BlueCross BlueShield. Under the BlueCard program, a provider treating a member from another Blue plan submits the claim to their own local Blue Cross Blue Shield plan, which then routes it to CareFirst BlueCross BlueShield for processing.
Arrangements vary by contract, and self-funded (ASO) accounts can differ — confirm with your local Blue plan before filing. See the BlueCard Program Provider Manual for the full rules.
HMO (Health Maintenance Organization) plans require members to choose a primary care physician (PCP) and obtain referrals before seeing specialists. Out-of-network services are generally not covered except in emergencies. Claims must be submitted to the payer listed above and should include referral authorization when applicable.
Under this POS prefix, the member can access care both in-network and out-of-network, with cost-sharing that varies based on how the care is accessed. In-network care coordinated through the member's PCP has the lowest out-of-pocket cost. Include referral documentation when billing at the in-network benefit level.
This prefix is associated with a High-Deductible Health Plan. HDHP members pay more out-of-pocket before insurance coverage begins. These plans are commonly paired with HSA or HRA accounts. When billing, be aware that the member may have significant cost-sharing responsibility until their annual deductible is satisfied.
