What PrefixLookup is

PrefixLookup is a free reference tool that maps Blue Cross Blue Shield alpha prefixes to the plan that processes their claims and shows billing details for that plan. It is run independently. It is not affiliated with, endorsed by or sponsored by the BlueCross BlueShield Association or any Blue Cross Blue Shield company, and Blue Cross, Blue Shield and the Blue plan names are trademarks of their respective owners, used here only to identify them. By using the site you agree to these terms.

Reference only

The information on this site is provided for general reference. It is not billing, coding, legal or financial advice, and it does not replace eligibility verification, the payer’s own provider resources, or your contract with a plan. Payer IDs, phone numbers, claims addresses and filing deadlines change, sometimes without notice. Confirm the details with the plan before you submit a claim, and treat the figures here as a starting point rather than the final word.

No guarantee of accuracy

We work to keep the data current and correct, and the methodology page explains how. Even so, we cannot guarantee that any value is accurate, complete or up to date at the moment you read it. Values marked as unverified are shown because they are the best information we have, not because they have been confirmed.

Limitation of liability

To the fullest extent permitted by law, PrefixLookup and the people who run it are not liable for any loss or damage arising from your use of the site or reliance on its contents, including denied, delayed or misrouted claims, missed filing deadlines, lost revenue, or any indirect or consequential loss. Nothing in these terms limits liability that cannot be limited by law.

Acceptable use

The site is for individual lookups by people and organizations working with BCBS claims. You may not scrape, crawl, copy or extract the dataset in bulk, whether by automated means or otherwise, and you may not reproduce substantial parts of it on another site or in another product. Search engines and similar services may index the site in the ordinary way. If you need the data in bulk or through an API, the pricing page explains how to ask; we license that separately.

Reporting errors

If you find a value you believe is wrong, use the report-a-problem link on the page or contact us. We check reports against the plan’s own resources before changing anything, and corrections are recorded on the changelog.

Changes to these terms

We may update these terms from time to time. The date at the top shows when they last changed. Continued use of the site after a change means you accept the updated terms.
